How to Fill Out and Submit the Smith Warranty Claim Form

To file a warranty claim with Smith Optics, go to smithoptics.com/en-us/pages/warranty and complete the Smith warranty claim form, which walks you through selecting your product type, identifying the defect, uploading photos, and attaching proof of purchase.1Smith Optics. Smith Optics Warranty Policy – Section: Where to Make a Claim The form is available to customers in the United States and Canada and handles sunglasses, goggles, helmets, prescription eyewear, accessories, and apparel through a single intake.

Gather Everything Before You Open the Form

The form doesn’t save your progress, so collect your materials first. You’ll need:

  • Your product’s model name. The form uses dropdown menus by category, then narrows to specific models. Check inside the frame arm, the helmet liner, or the original packaging if you’re not sure.
  • Proof of purchase. Smith’s U.S. warranty policy requires it, so have a digital copy of the receipt, order confirmation email, or invoice ready to upload.2Smith Optics. Warranty Policy
  • Month and year of purchase. This matters most for helmets (three-year window) and apparel (one-year window).
  • At least two photos: one of the whole product and one close-up of the defect. An optional third slot is available. Each file can be up to 10 MB.3Smith Optics. Smith Optics Warranty Claim Form
  • The product SKU if you have it from packaging or an order confirmation. Not required, but it helps Smith identify the exact item.

Confirm Your Product Is Actually Covered

The warranty protects against manufacturing defects, meaning flaws that trace to how the product was made rather than how it was used. Coverage length depends on the category:

  • Sunglasses and goggles, including custom goggles, are covered for the lifetime of the product against manufacturer’s defects.
  • Helmets are covered for three years from the date of purchase.2Smith Optics. Warranty Policy
  • Apparel and accessories are covered for one year from the date of purchase.4Smith Optics. Warranty Policy for Smith Optics New Zealand

“Lifetime of the product” refers to the functional lifespan of the eyewear, not the owner’s lifetime. Delaminated frames, hinges that snap under normal use, or vents separating from a goggle housing are the production failures the warranty is meant to address.4Smith Optics. Several situations fall outside coverage, and filing anyway wastes your time. Products that have been dropped, sat on, stepped on, run over, shattered, or chewed by a pet are excluded, along with any damage from misuse, abuse, or negligence.5Smith Optics. Warranty Policy Lens scratches are treated as wear, not defects. Normal wear and tear is excluded across all categories, and for apparel the natural breakdown of colors and materials over time is excluded even inside the one-year window.

Modifications void the warranty entirely. Aftermarket lenses, third-party nose pads, or any change to frame parts takes the product outside coverage, even if the defect you’re claiming has nothing to do with the modification. The warranty is limited to the original purchaser, and lost or stolen products are not covered.

Working Through the Form

Personal Information

Enter your first name, last name, email, phone number, and shipping address. The address is where Smith will send any replacement, so use one where you can reliably receive a package. Select your country, United States or Canada, from the dropdown to populate the correct address format.3Smith Optics. Smith Optics Warranty Claim Form

Product Type and Model

Choose your product type from the first dropdown: Sunglasses, Goggles, Helmets, Prescription (Rx), Accessories, or Apparel. A second dropdown appears for the specific model. For goggles and helmets, you’ll pick bike or snow first, then the model. For prescription eyewear, you’ll choose between Rx Sunglasses and Rx Eyeglasses, then the frame. Prescription claims go through this form rather than through a separate optical provider.

Warranty Reason and Description

Once you’ve selected a product, a “Warranty Reason” dropdown appears with defect categories specific to that type. Pick the closest match, then use the free-text description field to explain the problem specifically. “Left temple arm hinge cracked at the pivot point during normal folding” tells a reviewer more than “frame broke,” and detail here reduces the odds of follow-up questions.

Uploads

The form has four upload slots:3Smith Optics. Smith Optics Warranty Claim Form

  • Product Image. A full shot showing the entire product and overall condition. Photograph it on a clean, well-lit surface.
  • Product Defect. A close-up where the specific issue is clearly visible. Use natural lighting, avoid flash glare on lenses, and get close enough that the crack, delamination, or separation is obvious.
  • Proof of Purchase. A photo or screenshot of your receipt, order confirmation, or invoice.
  • Additional Photo. Optional. Use it for a second angle of the defect or other supporting image.

Each upload accepts files up to 10 MB. Resize phone photos first if they run larger, or the form will reject them.

What Happens After You Submit

Submitting the form sends your claim to Smith’s warranty team for review. Smith may ask you to return the defective product for physical inspection before finalizing the claim, so hold on to the item until your claim is fully resolved. Don’t throw it away.

If the claim is approved, Smith issues a warranty coupon redeemable toward a replacement. That coupon expires 90 days after it’s issued, so use it promptly.2Smith Optics. Warranty Policy Smith does not publicly state a processing timeline, so response times may vary with claim volume.

How to Avoid a Denied Claim

The most common rejection reasons are preventable. Submitting without proof of purchase is the fastest route to a denial, since Smith needs to verify you’re the original buyer and that the product is still within its coverage window. Search your email for the order confirmation if the paper receipt is gone.

Blurry or poorly lit defect photos are the next common issue. The reviewer can only see what you show. If the defect is a hairline crack, photograph it from an angle where it catches the light. A photo that clearly shows the problem outweighs a long written description.

Be honest about the cause. A frame cracked by a drop is accidental damage, not a manufacturing defect, and filing for impact damage wastes everyone’s time. A frame that cracked spontaneously during normal use, or while gently folding the temples, is the kind of failure this warranty exists for.
