Before you can register for in-person classes at any University of Hawaiʻi campus, you need to complete the UH TB screening form, have it signed by a U.S.-licensed healthcare provider, and upload it through Med+Proctor. The form is part of the UH system-wide health clearance process, and until it clears, the system blocks you from enrolling in anything that meets on campus.1University of Hawaiʻi. Health Clearance
Who Has To Submit the Form
Hawaiʻi Administrative Rules Chapter 11-157 requires every student at a post-secondary institution in the state to meet health clearance requirements, including a TB examination, before enrolling in any course of study longer than six months.2Hawaii Department of Health. Hawaii Administrative Rules Title 11 Chapter 157 – Exhibit C That covers undergraduates, graduate students, and post-baccalaureate unclassified students across all UH campuses.1University of Hawaiʻi. Students taking only online courses who will not set foot on any UH campus are exempt. The exemption disappears the moment you register for an in-person class or need access to campus facilities, and the form must be cleared before you attend.1University of Hawaiʻi. Health Clearance
Where To Get the Form
Download the current UH Health Clearance Form from hawaii.edu/health-clearance/. Print it, bring it to your medical provider, and have them fill in the clinical portions, sign each section, and return the completed document to you for upload. A separately signed record from a U.S.-licensed practitioner that includes all the required information and dates can be submitted instead of the UH-specific form.1University of Hawaiʻi. Health Clearance
The signing provider must hold a U.S. license as an M.D., D.O., Nurse Practitioner, or Physician Assistant. A clinic stamp also satisfies the requirement. Providers licensed outside the United States cannot sign the form, with one exception noted below for international students.1University of Hawaiʻi. Health Clearance
Filling Out the TB Risk Assessment
The form opens with a short questionnaire your provider walks through with you. The answers determine whether you need testing at all, so take it seriously.
The provider first checks for significant TB symptoms, defined by the Hawaiʻi Department of Health as a cough lasting three weeks or more combined with at least one of the following: coughing up blood, fever, night sweats, unexplained weight loss, unusual weakness, or fatigue. If significant symptoms are present, you go straight to further testing including a chest x-ray.3Hawaii Department of Health. TB Risk Assessment for Adults and Children
If there are no symptoms, the provider moves to the risk factor questions:3Hawaii Department of Health. TB Risk Assessment for Adults and Children
- Were you born in a country with an elevated TB rate? (Essentially anywhere other than the U.S., Canada, Australia, New Zealand, or Western and Northern European countries.)
- Have you traveled to or lived in a high-TB-rate country for four weeks or longer?
- Have you ever been in contact with someone who had infectious TB disease?
- Do you have a health condition affecting your immune system, or are you taking immunosuppressive medication?
If every answer is “No,” your provider can issue TB clearance without any testing. If any answer is “Yes,” a TB skin test or blood test is required.
If You Need a TB Test
When testing is required, your provider performs either a Mantoux tuberculin skin test (TST) or an Interferon-Gamma Release Assay (IGRA) blood test. UH accepts both.
Mantoux Skin Test
The provider injects a small amount of tuberculin into the skin of your forearm. You must return to the same provider between 48 and 72 hours later to have the result read; miss that window and you will likely need to start the test over. The provider measures the raised area (induration) in millimeters and records the exact number on your form, not a “positive” or “negative” label.4Centers for Disease Control and Prevention. Mantoux Tuberculin Skin Test
One timing issue catches students off guard. If you recently received a live vaccine such as MMR or Varicella, you must wait four weeks before taking a TB skin test or blood test. If your enrollment deadline is tight, your provider can perform a chest x-ray instead to preliminarily satisfy the TB requirement while you wait for the testing window to open.5University of Hawaiʻi. Frequently Asked Questions Regarding Health Clearance
IGRA Blood Test
The blood draw happens in a single visit with no follow-up reading, which is more convenient for students on tight schedules. Your provider records the date of the blood draw and the lab result on the form.1University of Hawaiʻi. Health Clearance
If Your Test Comes Back Positive
A positive skin test (induration of 10mm or greater) or a positive blood test does not block enrollment. It triggers one more step: a chest x-ray. Written documentation of a negative chest x-ray result must be submitted before your first day of attendance.2Hawaii Department of Health. Hawaii Administrative Rules Title 11 Chapter 157 – Exhibit C If the x-ray is also abnormal, you will need to work with your provider and potentially the Hawaiʻi Department of Health for further evaluation and treatment before clearance can be granted. Build extra time into your enrollment timeline if you have a history of positive TB tests.
International Students
International students face the same TB clearance requirement, with a documentation twist. All TB risk assessments, skin tests, and chest x-rays must be completed and signed by a U.S.-licensed healthcare professional. Testing done abroad by non-U.S.-licensed providers is generally not accepted. The one notable exception: Quantiferon Gold (IGRA) blood test results from outside the United States are accepted by UH.1University of Hawaiʻi. Health Clearance
If you are arriving from overseas and know your risk assessment will likely require testing, getting a Quantiferon blood test before you leave home can save significant time. Otherwise, plan to complete screening promptly after you arrive, since you cannot register for in-person classes until clearance is verified.
Submitting the Form Through Med+Proctor
UH processes every health clearance document through Med+Proctor. Submissions by email, fax, or any other method are not accepted.6Med+Proctor. How to Submit My Documents to Med+Proctor
- Create an account at medproctor.com using your UH student email address. Personal email addresses are not accepted. Some campuses use a Secured Sign-On that redirects you to log in with your UH portal credentials.6Med+Proctor. How to Submit My Documents to Med+Proctor
- Scan or photograph your signed form and upload it as a PDF, JPEG, or PNG. HEIC files and password-protected documents are rejected.6Med+Proctor. How to Submit My Documents to Med+Proctor
- Wait for verification. Standard processing takes about 48 hours; an expedited review within one hour is available for a fee from your account status page. Submitting records is otherwise free.6Med+Proctor. How to Submit My Documents to Med+Proctor
Make sure your scan captures all four corners of the page and that the provider’s signature and any clinic stamp are legible. Blurry or cropped uploads are the most common reason for processing delays.
Deadlines and What Happens if You Miss Them
All health clearance documentation must be submitted and verified before the first day of the semester. If your records are not cleared in time, UH restricts you to online-only courses until the clearance goes through.1University of Hawaiʻi. Health Clearance Until the hold is lifted, you cannot register for any in-person class.7University of Hawaiʻi. Health Clearances – MyUHINFO
Once Med+Proctor verifies your documents, the hold is removed and you get an email confirmation.8Hawaii Community College. Health Clearance Requirements If your submission is rejected, usually for an illegible signature, missing provider credentials, or an incomplete risk assessment, you will get a notification explaining what to fix. Check your Med+Proctor status page regularly in the weeks before the semester starts so you have time to correct problems.
Do You Need a New Test or Does an Old One Count?
Your Mantoux skin test must have been administered within 12 months before you first attend a post-secondary school in Hawaiʻi.2Hawaii Department of Health. Hawaii Administrative Rules Title 11 Chapter 157 – Exhibit C A TB clearance obtained at age 16 or older does not expire for college attendance, so if you received one during high school after turning 16, it remains valid when you enroll in the UH system later.9Hawaii State Department of Health.
