To complete the 1199SEIU enrollment form, download the version that matches your benefit fund from 1199seiubenefits.org/forms, fill in your personal and employer information exactly as it appears on your Social Security card, attach copies of birth or marriage certificates for any dependents, sign and date every required line, and submit it online, by mail, or in person.11199SEIU Funds. Forms for Members The form registers you as a member of 1199SEIU United Healthcare Workers East and connects you to the health, dental, and pension benefits run by one of six benefit funds.
Pick the Right Version of the Form
There is no single enrollment form. The 1199SEIU system runs six benefit funds, and each one has its own version. Your fund is determined by where you work and the collective bargaining agreement covering your job, not by choice. Separate forms also exist for combined benefits-and-pension enrollment and for pension-only enrollment, so look carefully at the title on the PDF before downloading.
If you are not sure which fund applies, ask your union delegate or your employer’s HR department, or call the Benefit Funds at (646) 473-9200.21199SEIU Funds. Contact Us Submitting under the wrong fund delays everything, so confirm this first.
When Your Coverage Actually Starts
Filing the form does not switch benefits on immediately. Eligibility generally requires that you have worked for a contributing employer for at least 90 days and that the employer has made contributions to your fund for at least 30 days.31199SEIU Funds. Eligibility for Health and Other Benefits for Greater New York Members You can send the form in before you hit those thresholds, but coverage begins only once both are met. Your employer’s payroll department can tell you when contributions started.
Most members pay little or nothing out of pocket for health coverage, though some funds involve modest co-pays and premiums.41199SEIU Funds. Healthcare
What to Have Ready Before You Start
Half-finished forms get held up. Put everything on the table before you pick up a pen:
- Your Social Security number and date of birth.
- A government-issued photo ID. Your legal name on the form has to match it exactly.
- Your current mailing address and phone number. Your ID card and all benefit mail will come to the address you list.
- The full name and address of your healthcare facility, not just the parent system.
- Your official job title and start date, since different titles fall under different collective bargaining agreements.
- For each dependent: Social Security number, plus a marriage certificate for a spouse or a birth certificate for a child.
Send clear copies of legal documents. Never send originals.
Filling Out Each Section
The form moves from your personal information to your employer details to dependents and beneficiaries. A few spots cause most of the rejections.
Personal and Employer Information
Print your legal name exactly as it appears on your Social Security card. A missing middle initial or a hyphen dropped from a last name is enough to trigger a verification hold. Double-check the Social Security number digit by digit. For employer information, list the full name and address of the facility where you work. Your union delegate can give you the employer’s fund contribution ID if the form asks for it.
Adding Dependents
Eligible dependents are your legal spouse (regardless of gender) and your biological or adopted children. Domestic partners are not eligible for coverage through the Benefit Fund.51199SEIU Funds. Frequently Asked Questions You need a Social Security number for each dependent, because health insurers are required to report that information to the IRS on Form 1095-B under the Affordable Care Act.6Internal Revenue Service. Questions and Answers About Reporting Social Security Numbers to Your Health Insurance Company
Attach a copy of your marriage certificate for a spouse and a copy of each child’s birth certificate listing you as a parent. If a document is in another language, include an English translation and a signed certification from the translator stating they are competent in both languages and that the translation is accurate.7U.S. Department of State. Beneficiary Designations
The beneficiary section applies to life insurance and pension payouts. For each beneficiary, write their full name, date of birth, Social Security number, and relationship to you. If you want to name a minor child, keep in mind that a court-appointed guardian may need to be in place before the fund can release payment to that child.8U.S. Office of Personnel Management. If My Child Is Not Yet of Legal Age, Do I Have to Appoint a Legal Guardian if My Child Is My Beneficiary? Naming an adult as a contingent beneficiary avoids that complication.
Health and Pension Checkboxes
If you downloaded the combined Benefits and Pension form, both sections appear together. If your fund uses a pension-only form, health enrollment is handled separately. Check every option that applies. A blank checkbox you meant to tick becomes a coverage gap that takes real time to fix. Sign and date everywhere the form requests it. An unsigned form is returned, not processed.

Whichever route you choose, send photocopies of birth and marriage certificates and keep the originals.
After You Submit
Staff will cross-reference your employment records against payroll contributions from your employer. If anything is missing or unreadable, you will get a call or letter. The usual holdups are mismatched names, missing Social Security numbers, unsigned signature lines, and omitted supporting documents.
Once your enrollment is verified, your health insurance ID card arrives by mail. Keep a copy of your submitted form in the meantime. You can check status in the MyAccount portal.91199SEIU Funds. 1199SEIU MyAccount If weeks pass with no response, call (646) 473-9200. A short phone call usually clears a stuck file faster than waiting.
Updating Your Information Later
You do not need a brand-new form for every change. Through MyAccount you can update your address, add a dependent after a marriage or birth, coordinate spousal benefits, and request a replacement ID card.91199SEIU Funds. 1199SEIU MyAccount Dependent additions still require the same supporting documents, including the marriage or birth certificate and the dependent’s Social Security number. Report changes promptly, because outdated records lead to claim denials that are tedious to appeal later.
