The Babe Ruth League Consent for Treatment form is a one-page PDF that lets a league representative seek immediate medical care for your child if you cannot be reached during a game, practice, or tournament. You download it from the national league’s website, fill in player, contact, medical, and insurance details, sign it as a parent or guardian, and hand it to your team manager before your child participates in any sanctioned activity. Most of it takes minutes. The insurance section is where parents most often get stuck.
Where To Download the Form
The form is posted on the Babe Ruth League national website at baberuthleague.org under Resources or Forms.1Babe Ruth League. Babe Ruth League It is a writable PDF, so you can type into the fields on a computer before printing.2Babe Ruth League. Babe Ruth League Consent for Treatment Form Your local league usually also hands out paper copies at registration. The same form covers every Babe Ruth program.
What Each Section Asks For
Have your child’s insurance cards and your family doctor’s phone number in front of you before you start. The fields fall into three groups.
Player and contact information. You list the player’s full name, age (not date of birth), home address, city, and state.2Babe Ruth League. Babe Ruth League Consent for Treatment Form There are three phone fields: daytime, home, and cell. Put numbers where you or another responsible adult can actually be reached during games and practices. These numbers are what the league will call first if your child is hurt.
Medical information. You give your child’s family physician and that office’s phone number. Two separate fields follow, one for allergies and one for medications your child takes regularly. Be specific. Write the drug name rather than “inhaler” or “allergy pill.” If there is nothing to list, write “none” instead of leaving the line blank, so the field reads as answered rather than skipped.
League and insurance information. You enter the name of your local Babe Ruth affiliate, then two separate sets of insurance details:
- The league’s accident insurance carrier and policy number. This information applies to the whole team and comes from the league’s chartering paperwork, not from anything you own. If the fields are blank, your team manager or league president has what you need.
- Your family health insurance company and policy number, copied from your member ID card.2Babe Ruth League. Babe Ruth League Consent for Treatment Form
The form asks for both because both come into play if your child is injured. More on that below.
Who Signs, and Whether It Needs To Be Notarized
A parent or legal guardian signs and dates the bottom of the form for any player under 18. A player who is 18 or older can sign for themselves.2Babe Ruth League. Babe Ruth League Consent for Treatment Form Notarization is not required. There is no notary line on the form and no language calling for one. A handwritten signature and the date are enough.
The PDF has no built-in digital signature field. It is meant to be printed and signed by hand. Some local leagues that use online registration platforms may collect the consent electronically during sign-up, so if you are not sure whether a paper copy is still expected, ask your local administrator.
When and Where To Turn It In
Give the signed form to your team manager. At a minimum, it must be completed and in the manager’s possession before state-level tournament play begins.3Babe Ruth League. How to Assemble Your Tournament Credentials In practice, most local leagues collect it much earlier, at registration or before the first practice. A player without a signed form on file sits out until the paperwork is done.
For tournament play, the manager keeps the consent forms in a credentials binder that travels with the team and stays at the field, so medical information is on hand if a player is hurt.3Babe Ruth League. How to Assemble Your Tournament Credentials Managers and coaches should treat the forms as private medical records.
What You Are Authorizing
By signing, you authorize a Babe Ruth League representative to use their judgment in obtaining immediate medical care for your child in case of an accident or illness.2Babe Ruth League. Babe Ruth League Consent for Treatment Form That language is broad enough to cover calling an ambulance, authorizing an emergency room visit, or taking whatever step a reasonable person would take when a child is injured and the parent is not yet on scene.
The consent does not hand the league authority over routine or ongoing medical decisions. It covers the gap between the moment something goes wrong and the moment you arrive or are reached by phone, which is why the contact numbers on the form matter as much as the signature.
Why the Form Asks for Two Insurance Policies
Every chartered Babe Ruth League carries a secondary accident policy through K&K Insurance Group that pays up to $250,000 in accident medical benefits per participant, subject to a $100 per-person deductible.4Babe Ruth League. Babe Ruth League Chartering/Insurance It covers approved league activities, including practices, regular-season games, and tournaments.
The important word is secondary. The league policy pays only after your family’s own health plan has processed the claim. If you have no other health coverage, the league policy acts as primary, still subject to the $100 deductible. That is the reason the consent form asks for both sets of insurance details on the same page: if your child is hurt, both insurers may be billed, in order.
Filing a Claim If Your Child Is Injured
If an injury happens during a league activity, the claim form has two parts. Part I is completed and signed by an authorized league official. Part II is completed and signed by the parent or guardian.5K&K Insurance. Babe Ruth League, Inc. Medical Claim Form A claim submitted without the league official’s signature will be returned, so coordinate with your manager or league president before mailing anything.
Along with the form, you submit itemized bills from every provider showing the patient’s name, diagnosis, treatment, dates, and charges. If you have other health insurance, bill your own carrier first and include copies of the explanation of benefits statements with the claim to K&K. If your insurer denies the claim, include the denial letter.
