To apply for the Dexcom Patient Assistance Program, start at assistance.dexcom.com, where you and your healthcare provider complete a two-part application: a patient section with proof of income and a physician section confirming your diabetes diagnosis. The program provides free continuous glucose monitoring supplies to people who are uninsured or whose commercial insurance does not cover CGM, provided household income is at or below 400% of the Federal Poverty Level and you live in the United States or its territories.
Do You Qualify
Three things have to be true. You need to meet all of them.
- Income. Household income at or below 400% of the Federal Poverty Level. For 2026 in the 48 contiguous states, that’s $63,840 for a single-person household and $132,000 for a family of four. Alaska and Hawaii use higher thresholds; 400% FPL for a single person in Alaska is $79,800.1U.S. Department of Health and Human Services. 2026 Poverty Guidelines
- Insurance. You must either have no insurance, or have a commercial plan that specifically does not cover continuous glucose monitors. A plan that covers CGM but leaves you with a high copay does not qualify you for the PAP.2RxAssist. Dexcom, Inc. – Dexcom Patient Assistance Program
- Residency. Legal resident of the United States or its territories.2RxAssist. Dexcom, Inc. – Dexcom Patient Assistance Program
If you’re enrolled in Medicare, Medicaid, TRICARE, or another federal healthcare program, you cannot receive free products through the PAP. The federal Anti-Kickback Statute prohibits manufacturers from giving free items to patients whose care is reimbursed by government programs.3Office of Inspector General. What to Gather Before You Start
The application has a patient side and a physician side. Have everything ready before you open the portal so the process doesn’t stall halfway through.
Your Documents
You need proof that your household income falls within the threshold. The cleanest option is your most recent federal tax return (Form 1040). If you haven’t filed recently, recent pay stubs or a Social Security Benefit Statement can substitute. Financial attachments are required, so plan on uploading or sending them with the form.2RxAssist. Dexcom, Inc. – Dexcom Patient Assistance Program
Match your name and contact details to your legal ID exactly. A missing middle initial or an old address can hold up the review.
Your Provider’s Section
Your healthcare provider completes the medical portion, confirming your diabetes diagnosis and the clinical need for continuous glucose monitoring. The provider typically supplies their National Provider Identifier (NPI) and signs the form. Because the clinical information needs to be current, don’t have your doctor sign months in advance. Send the full application within a few weeks of getting the signature.
Endocrinology offices often keep these forms on hand and know the workflow. A primary care provider managing your diabetes can complete the section too.
How to Submit
Dexcom runs a dedicated portal at assistance.dexcom.com. It’s the most direct way in, though it requires JavaScript and works as a web application rather than a downloadable PDF. If you can’t use the portal, ask your provider’s office whether they can access the form for you, or contact Dexcom to ask about other submission options.
Put your name and date of birth on every page of supporting documents. If pages get separated during processing, that keeps them matched to your file. For anything you fax or mail, use a trackable method so you have proof of delivery.
After You Apply
Dexcom does not publish a guaranteed turnaround. Incomplete forms and illegible documents trigger requests for more information, which pushes back the review. The two most common holdups are missing financial attachments and unsigned physician sections, so check both before you submit.
Once you’re approved, sensors and related supplies ship directly to your home rather than through a pharmacy. Your account is tied to the program for a set enrollment period, and you’ll receive authorization confirming it.
Re-Enrolling Each Year
Enrollment runs through the end of the calendar year no matter when you were approved. If you were approved in March, you still need to re-enroll before January to keep receiving supplies.2RxAssist. Dexcom, Inc. – Dexcom Patient Assistance Program Re-enrollment is a fresh application with updated financial documentation and a new physician signature.
Start at least a month before year-end so there’s no gap between shipments, and book the provider appointment early. If your income or insurance status changes mid-year, notify the program; continued eligibility depends on still meeting all three criteria.
If You Don’t Qualify
If you have commercial insurance but face high out-of-pocket costs, Dexcom runs a separate pharmacy savings program with coupons that reduce the cash price of sensors and receivers.4Dexcom. CGM Cost and Insurance Coverage That’s a different track from the PAP, which is built for people with little or no insurance.
If you’re on Medicare or Medicaid and your plan denies CGM coverage, the usual path forward is an appeal through the plan or a coverage exception requested by your provider. Government plans have expanded CGM coverage in recent years, so a denial isn’t always the last word.
