The Jack and Jill Legacy Membership Request Form is a one-page application that lets an adult who graduated from a Jack and Jill of America youth chapter join the organization through the legacy pathway. You download it from jackandjillinc.org, type in your information and your mother’s membership history, collect three signatures, and route it through your mother’s former chapter president so it reaches the National Office by November 15.1Jack and Jill of America, Incorporated. Confirm You’re Eligible Before You Start
Legacy status applies to a child, male or female, who graduated from a Jack and Jill chapter and whose mother was a member in good standing at the time of that child’s graduation.2Jack and Jill of America, Inc. Membership If your mother has passed away, the organization treats her as having completed her tenure in good standing, so eligibility is preserved.3Jack and Jill of America, Incorporated. The Legacy Process
You can use the legacy pathway only once. If you join as a legacy and later resign or are terminated, you cannot re-enter through legacy again.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form You also need at least one child in the youth program’s age range (2 through 19) to participate as an active member.
Gather What You’ll Need Before You Open the Form
Pulling the following together first will keep you from stopping mid-form:
- A physical address (no P.O. boxes are accepted).
- The chapter you’re requesting to join, based on where you currently live rather than your mother’s old chapter, and the Jack and Jill region that covers it.
- Your mother’s last chapter name, state, and region, plus her initiation year and last year of active membership.
- Each of your children’s name, date of birth, gender, and current grade.
- Verification of your mother’s good-standing tenure if the chapter cannot confirm it from its own records.
For that last item, the form accepts three types of backup: a copy of your mother’s “Completion of Tenure” form; a notarized letter from a financial or former member who held membership at the same time as your mother and can personally attest to her tenure; or a notarized letter from another Jack and Jill member who has direct knowledge of her service in the chapter.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form If you don’t supply any of these and the chapter can’t verify your legacy, headquarters will research its own files, which adds time to processing.2Jack and Jill of America, Inc. Complete the Form Section by Section
The entire form must be typed. Handwritten forms are not accepted.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form
Your Information
The first section asks for the basis of your legacy claim (whether you are the legacy or the spouse of a male legacy), your name, physical address, phone, email, the chapter you’re requesting, the region, and your spouse’s name. The spouse field is required regardless of which spouse holds the legacy.
Legacy and Mother’s Information
The second section establishes the membership chain. Enter the name of the legacy (the person who actually graduated from the youth program), the mother of the legacy and whether she is living or deceased, her last chapter affiliation with state and region, and her initiation and last active year. Then list each of your children with name, date of birth, gender, and current grade.
Collect the Three Required Signatures
Three signatures must appear on the form before it can go to the National Office. You sign and date the acknowledgment section, which confirms you understand that approval cannot be deferred and that falsifying information carries consequences. Your legacy mother signs and dates the form; if she is deceased, that line is left blank. The chapter president of your mother’s last chapter signs to approve the application and also completes a short verification section confirming whether your mother completed tenure in good standing and whether she is currently an Associate or Life Member.1Jack and Jill of America, Incorporated. How and When to Submit
You do not submit the form yourself. After signing it, forward it to the current president of the chapter from which you graduated.2Jack and Jill of America, Inc. Membership Once that president reviews and signs, either the Chapter President or Chapter Vice President submits it to the National Office using the submission link at the bottom of the document.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form
The form must reach the National Office by November 15. Anything that arrives later will not be processed for that program year’s intake and will have to wait a year.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form Because the chapter president’s signature is a prerequisite, contact that chapter well before November so there’s time to verify records and sign.
What Happens After You Submit
The National Office takes 60 to 90 days to process a completed form.3Jack and Jill of America, Incorporated. The Legacy Process If you’re approved, you must be initiated by May 31 of that program year. There is no deferral option, and the acknowledgment you signed on the form locks you into that timeline, so only apply when you’re ready to join that year.1Jack and Jill of America, Incorporated. Falsifying any information on the application can result in revocation of the legacy request, or termination of your membership if you have already been initiated.1Jack and Jill of America, Incorporated. Jack and Jill Legacy Membership Request Form
If You’re a Male Legacy or the Spouse of One
Jack and Jill is a mothers’ organization, so a male graduate of the youth program does not become an active member himself. His female child’s mother, or his spouse, is entitled to become a member automatically upon submitting an application and payment, provided they have a child who is at least two years old at the time of installation.3Jack and Jill of America, Incorporated. The Legacy Process On the form, that spouse is the one who fills out “Your Information” and indicates she is applying as the spouse of a male legacy.
One limit applies: if a male legacy’s first spouse used the legacy benefit and that marriage ends, any later spouse cannot use the legacy pathway and must seek membership through the standard invitation process.3Jack and Jill of America, Incorporated. The Legacy Process
