How to Fill Out the Little League Tournament Player Verification Form

The Little League Tournament Player Verification Form is the document that clears a player for All-Star and postseason tournament play by confirming age, residency or school attendance, and league boundary. It is completed for each player, signed by a parent or legal guardian, the local League President, and the District Administrator, and carried with the team’s eligibility binder throughout the tournament.¹

Where the Form Comes From

There are two accepted versions. League officials with Data Center access generate the form as part of the online tournament affidavit, and the system pre-fills league name, league ID, and the player’s information already on file. A blank PDF is also posted on Little League International’s website for leagues that prefer to complete it by hand.¹ Most leagues use the Data Center route because it cuts down on transcription errors, but either is valid.

One affidavit is created per team enrolled in tournament, and each affidavit generates individual verification forms for up to fourteen players, or sixteen in Senior League.²

Documents to Have in Front of You

Before writing anything on the form, pull together the supporting records. Every entry on the verification form has to match these documents exactly, so having them in hand prevents the most common cause of delay at District Administrator review.

Proof of Age

A government-issued birth certificate is the standard proof-of-age document. If you cannot get one, the District Administrator can issue a Statement in Lieu of Acceptable Proof of Birth after reviewing alternative supporting documents. When that route is used, the statement and its supporting documents must travel in the team’s eligibility binder throughout tournament play, and the District Administrator keeps a copy as well.³ Little League also partners with VitalChek for express birth-certificate ordering if you need one quickly.

Residency: One Document From Each of Three Groups

If your child qualifies through where the family lives, you need one document from each of three separate groups. Every document must show the parent or guardian’s full name, street address, city, state, and zip code, and must be dated or in force between February 1 of the previous year and February 1 of the current year.⁴

Group I: driver’s license, school records, vehicle registration or lease, employment records, or insurance documents.
Group II: welfare or child-care records, federal records such as tax documents or Social Security records, state records, local municipal records, support-payment records, homeowner or tenant records, or military records.
Group III: voter registration, utility bills (gas, electric, water, phone, heating, waste disposal), financial records, medical records, or internet, cable, or satellite records.

Two documents from the same group will not satisfy the requirement. You need exactly one from each.⁴

The School Enrollment Alternative

If the child attends a school physically within the league’s boundaries, you can skip the three-group residency requirement and use a single School Enrollment Form. A principal, assistant principal, or someone otherwise authorized to sign on the school’s behalf certifies that the student enrolled and was attending before October 1 of the current academic year.⁵ The form is completed only once during a player’s Little League career unless the child changes schools, in which case a Regulation II(d) waiver would be needed.⁶

League Age Is Not the Child’s Current Age

League age is calculated from a fixed cutoff. For Little League baseball, a player’s league age is the age they are on August 31 of the current year.⁷ For softball, the determination date is December 31 of the previous year. Players league age 8 through 16 are eligible for selection to an International Tournament team as long as they meet the residency or school attendance requirements.⁸

The birth date on the verification form must match the birth certificate exactly. A transposed digit is enough to hold up review.

Filling In the Form

Whether you use the Data Center version or the PDF, the form collects the same core information:¹

League name and League ID, found on the league’s charter or in the Data Center dashboard.
Player’s full name, matching the birth certificate exactly.
Date of birth, transcribed directly from the birth certificate or Statement in Lieu document.
Proof-of-age document type, indicating whether a birth certificate or an approved alternative was presented.
Residency proofs or school attendance: list the three residency documents (one per group) or note that the School Enrollment Form is being used.
Applicable waivers: if the player is competing under a Regulation II(d) waiver or a Charter Committee waiver, note the type here.

Keep every supporting document in front of you as you fill the form in. Names, addresses, and dates need to line up across all of them. A mismatch between the form and a residency document is one of the fastest ways to get flagged.

A separate section is reserved for league officials. When building the affidavit in the Data Center, officials enter each player’s regular-season team, the number of games played, and the address or school being used to establish eligibility.² The League President uses this section to confirm the player’s address or school falls within the boundaries the league has on file with the district.

If Your Child Moved or Changed Schools

A player who originally qualified through residency but has since moved or changed schools may still be eligible under a Regulation II(d) waiver. The rule allows a child to continue with the league as long as they previously lived within the boundary and played there, and it also covers cases where the league boundary itself was redrawn.⁹

There is no standardized application form for most waivers. The local league’s Board of Directors approves the waiver request, which is then written out with supporting documentation and submitted to the District Administrator. For a II(d) waiver specifically, proof-of-residency documents for the player’s previous address, the one that was inside the league boundary, must accompany the request.⁹ Once approved, the waiver type is recorded on the verification form so tournament officials can see why the player’s current address sits outside the boundary.

The Three Signatures

The completed form needs three signatures before a player is cleared for tournament competition.¹

The parent or legal guardian signs first, confirming the information provided is true and accurate. Little League warns that a false statement of residence or school attendance can lead to the player being declared ineligible.⁴
The League President reviews the documentation packet and signs, verifying that the player’s address or school is within the league’s approved boundaries.
The District Administrator conducts a final review of every birth certificate, residency document, and school enrollment form against the entries on the verification form, then signs to clear the player for postseason play.

Once all three signatures are in place, the verification form remains valid for the rest of that player’s Little League career. No new form is needed for future tournament selections as long as residency or school enrollment stays the same. If either changes, a new form and a II(d) waiver claim would be required.¹

What the Team Carries to Every Game

Individual verification forms do not travel alone. They are compiled into a tournament eligibility binder that the team manager brings to every game, along with several other required documents:

The Tournament Affidavit itself, the master document identifying all players, coaches, and the manager, generated through the Data Center and reprinted any time information is edited.²
One signed verification form per player on the roster.
Birth certificates or proof-of-age documents. Any player using a Statement in Lieu of Acceptable Proof of Birth must have that statement and its supporting documents in the binder.¹⁰
A printed copy of the league’s approved boundary map, generated separately from the Data Center.
Background check and training records for managers and coaches, including annual background checks and the required Abuse Awareness and Diamond Leader training.¹¹

The binder is presented to the District Administrator at a formal verification meeting before the team’s first tournament game. Keep a personal copy of your child’s signed verification form as your own proof of eligibility if questions arise in later seasons.

What Happens if a Player Is Found Ineligible

If an ineligible player is identified during a game, that player must be removed immediately. The opposing manager then decides whether to continue the game under protest or let it stand.¹² Penalties for using an ineligible player can include disqualification of the individual player, the entire team, or the league from both regular-season and tournament play.⁴ Leagues that fail to comply with background-check requirements for their volunteers face possible suspension or revocation of tournament privileges and their charter.¹³

Matching every document, confirming every date, and collecting all three signatures well ahead of the first tournament game is the simplest way to keep your child on the field for the whole All-Star run.
