The Qantas unaccompanied minor form, officially titled “Children Travelling Alone,” is a PDF you download from the Qantas website, complete in English, print, and bring to the airport on the day of travel.1Qantas. Qantas Unaccompanied Minor Form It records the child, every flight segment, the adult handing the child over, and the adult collecting them, so staff can verify identities at both ends of the trip. You can also pick up a paper copy at an airport check-in counter, but filling it out at home is faster and avoids mistakes under time pressure.
Every mandatory field is marked with an asterisk. Leave none of them blank. Write legibly in English, use the child’s legal name as it appears on the travel document, and match names on the form to the government-issued ID each adult will present.
Section-by-Section: What Each Field Needs
Child’s Details
Enter the child’s full legal name exactly as it appears on their travel document. For international flights, add the passport number. Include the Qantas booking reference from your confirmation.1Qantas. Qantas Unaccompanied Minor Form
Flight Information
Fill in the flight number, date, departure city, and arrival city for every segment. Each connecting leg gets its own row. Every segment listed must be a Qantas-operated flight; the service does not cover codeshare flights operated by other carriers, with narrow exceptions for Finnair-operated services QF291, QF292, QF295, and QF296, and Alliance Airlines flights carrying a QF flight number.2Qantas. Person Dropping Off the Child
Give the full name, address, mobile and work phone numbers, government-issued ID type, and ID number of the adult delivering the child to the airport. This person signs the form at the check-in counter when they hand the child over to Qantas staff.1Qantas. Qantas Unaccompanied Minor Form
Person Collecting the Child
The same details are required for the person meeting the child at the destination: full name, address, phone numbers, ID type, and ID number. They sign the form on collection.1Qantas. Qantas Unaccompanied Minor Form
Get the names right. Staff at the destination compare the name on the form against the pickup person’s photo ID, and the ID number has to match too. A nickname written in place of a legal name, or an ID number entered with a typo, can delay the handover or prevent release.
Who Can Be Listed as the Pickup Adult
The person collecting the child must be at least 18 years old, or a sibling aged 15 or older.2Qantas. Unaccompanied Minors and Young Passengers A travel companion aged 12 to 14 does not qualify, even on an adult fare. If the only person available to meet or accompany the child falls in that age range, the child still needs to be registered as an unaccompanied minor and a qualifying adult has to be named on the form.3Qantas. Who Needs to Use the Form
Qantas treats any child under 12 as an unaccompanied minor when they are not traveling in the same cabin as a parent, guardian, or sibling aged 15 or older. For children aged 5 to 11, registration is mandatory if no qualifying companion is on the booking. Children aged 12 to 15 can be registered optionally.2Qantas. Unaccompanied Minors and Young Passengers
Age minimums depend on the route. For Australian domestic flights and international flights under six hours, the child must be at least 5. For international flights of six hours or more, the minimum age is 6. Children younger than that cannot use the service and must travel with a fare-paying companion aged at least 15.2Qantas. Itinerary Rules the Form Assumes You’ve Met
Before you fill in the flight segments, confirm the booking actually complies with the unaccompanied minor rules. If it doesn’t, no amount of careful form-filling will fix it at the counter.
- No more than four hours between any two connecting flights.
- No overnight connections.
- The child cannot be booked on the last flight of the day on a route, unless it is the only flight that day.
- No cross-airport transfers in the same city (for example, Melbourne Tullamarine to Avalon, or London Heathrow to Gatwick).
- Every segment must be Qantas-operated, with the Finnair and Alliance Airlines exceptions noted above.
- No connections on other carriers at any point in the itinerary.

What to Bring Alongside the Form
Your child should carry paper copies of the completed form plus any required travel documents, such as a passport for international flights.2Qantas. Unaccompanied Minors and Young Passengers On Australian domestic flights, children under 18 traveling unaccompanied do not need their own photo ID; a Qantas customer service agent escorts the child through departure and arrival.4Qantas. Travel Documents
For travel to or from the United States, a signed consent letter from both parents is recommended. It should be notarized, written in English, and state that the child is traveling with your permission. Contact the embassy or consulate of the destination country for any additional entry requirements that apply to minors.5USAGov. International Travel Documents for Children
The drop-off adult should plan to arrive at least one hour before departure for domestic flights and two hours before departure for international flights, and must stay at the airport until the flight is in the air in case it returns to the gate.2Qantas. Unaccompanied Minors and Young Passengers
Health Eligibility
To use the service at all, the child must be able to eat and drink without assistance and recognize the need for and self-administer any prescribed medication. Cabin crew will not give medication to an unaccompanied minor. A child who carries an EpiPen or inhaler needs to know when and how to use it. Staff can help with small tasks like opening food packaging, and nothing more. A child who requires hands-on medical care cannot fly unaccompanied and needs a fare-paying guardian or carer aged 15 or older in the same cabin.2Qantas. Unaccompanied Minors and Young Passengers
Fees and How Booking Timing Affects Them
For Australian domestic flights, you can book the service online by creating a booking with no adults on the ticket; the fee is charged at checkout. International bookings and any itinerary with connections generally require a call to your local Qantas office or a travel agent.2Qantas. Unaccompanied Minors and Young Passengers
The fee is per child, per direction. Two siblings on the same flight each pay.
- Australian domestic, pre-booked: A$55 per child, inclusive of GST.
- Australian domestic, registered at the airport: A$110 per child.
- International, pre-booked: A$90 per child per direction.
- International, registered at the airport: an additional A$90 service fee on top of the standard A$90.

Each flight has a cap on how many unaccompanied minors can be supervised at once, so popular routes and school holiday periods fill up. Book early, pre-book the service when you buy the ticket, and have the form completed before you leave for the airport.
