The Summer Rising Supplemental Enrollment Form is the intake paperwork the community-based organization (CBO) at your child’s assigned site needs on file before the summer session begins. You fill it out after your child has already been accepted into Summer Rising through the MySchools portal, not as a way to apply. The form itself states it is “for participants who were accepted into the Summer Rising program through the centralized enrollment portal.”1NYC.gov. DYCD Summer Rising Supplemental Enrollment Form
What This Form Actually Does
Summer Rising runs in two halves of a day. NYC Public Schools staff lead the morning academics. A DYCD partner organization — the CBO — leads the afternoon enrichment at the same site.2Department of Youth & Community Development. Summer Rising 2026 The CBO operates separately from the school building’s academic staff and keeps its own records. Even though your child was already placed through the city’s centralized system, the CBO still needs its own intake paperwork for the enrichment portion: emergency contacts, health details, and permissions for activities and field trips.
The academic enrollment database and the DYCD enrichment system do not share a backend, so CBO staff cannot pull missing details from MySchools. Anything you leave blank becomes a phone call or an email exchange that can delay your child’s start.
Where to Get the Form
The CBO at your child’s assigned site provides the form, either in person at an orientation event or as a downloadable PDF. The English version is posted on the DYCD website.1NYC.gov. DYCD Summer Rising Supplemental Enrollment Form If your site handed you a paper copy at orientation, use that one, because some CBOs attach site-specific pages.
What to Have Ready Before You Start
Gather everything in one sitting so you aren’t hunting for a report card halfway through:
- Your child’s nine-digit OSIS number (also called the Student ID). It appears on report cards, student ID cards, and in your NYC Schools Account.3NYC Department of Education. New York City Schools Account (NYCSA)
- Your child’s current school name and grade level. The CBO uses these to confirm the placement matches the grade band the site serves.
- Emergency contact information: names, phone numbers, and relationship to the child for at least one authorized pickup person besides the parent or guardian.
- Medical and allergy information: known allergies, medications, and any conditions staff should be aware of during activities and field trips.
- Parent or guardian contact details: a working phone number and email address where the site can reach you during program hours.
Filling It Out Without Delays
Complete every field legibly. Blank fields and illegible handwriting are the two most common reasons a CBO has to chase a family down before day one. Double-check that the OSIS number matches the one on your child’s MySchools account exactly, since a transposed digit is enough to flag the record as a mismatch. If you have more than one authorized pickup person, list them all; the site will not release your child to anyone who isn’t named on the form.
Review the activity and field trip permissions carefully before signing. Enrichment programming includes outings away from the site, and a missing signature on a permission line can mean your child stays behind on a trip day.
If Your Child Has an IEP
The form behaves differently depending on whether your child is attending Summer Rising for the full day or only for IEP services.
Students enrolled in District 75, Extended School Year (ESY), or Autism programming who want only their IEP services — without the afternoon enrichment component — should not use the MySchools portal, and would not fill out the DYCD supplemental form, because they are not participating in CBO-led programming. For IEP services only, the family contacts the child’s school directly.4NYC Public Schools. Summer Rising 2026
If you want your child to stay for enrichment programming until 6 PM alongside their IEP services, you do apply through MySchools and you do complete the supplemental form once placed. Families in Autism, ESY, and D75 programming receive priority for Summer Rising seats.4NYC Public Schools. Summer Rising 2026 Use the medical and accommodations sections of the form to flag anything the CBO needs to know about for the afternoon portion of the day.
Students who receive a health paraprofessional or nurse during the regular school year through an IEP or 504 Plan receive those same services throughout the full Summer Rising day. The school prepares a Summer Rising Accommodation Plan for every student with an IEP and shares it with both the parent and the Summer Rising site before the session starts.4NYC Public Schools. Summer Rising 2026 The plan comes through this separate channel, so you don’t need to recreate it on the supplemental form.
If You Don’t Yet Have a Seat
The supplemental form is not an application and will not get your child into the program. Admission runs through MySchools. The 2026 application opened on March 3 and closed on March 27 at 11:59 PM, offers were released on April 21, and the deadline to accept was May 5.4NYC Public Schools. Getting Help
If something on the form doesn’t match what MySchools shows — a wrong grade, a misspelled name, an OSIS number you can’t locate — contact your child’s school first, because corrections there flow to MySchools.
