How to Fill Out the Supervised Experience Documentation Form: LPC Associate Upgrade

To upgrade from LPC Associate to full LPC in Texas, your board-approved supervisor completes the Supervised Experience Documentation Form for Texas LPC applicants, attesting to the hours you accumulated under their supervision. You then upload the signed form through the Texas Behavioral Health Executive Council (BHEC) online portal along with three other required items. There is no fee for the upgrade application, and your supervisor must submit accurate documentation to BHEC within 30 days of the end of your supervision or the completion of your hours, whichever comes first.1Texas Behavioral Health Executive Council. Check Your Hours Before Filling Anything Out

Texas requires at least 3,000 total hours of supervised experience, including a minimum of 1,500 hours of direct client counseling.3Texas Behavioral Health Executive Council. Professional Counselors – Applying for a License FAQs Those hours must be spread across no fewer than 18 months of practice, and every hour must have been earned after your LPC Associate license was issued.2Legal Information Institute. 22 Texas Administrative Code 681.93 – Supervisor Requirements

You also need at least four hours of individual or group supervision each month, with no more than half of your total supervision hours coming from group settings. You cannot have had more than two BHEC-approved supervisors at any one time during your Associate period.4Texas Behavioral Health Executive Council. Section-by-Section: What the Form Asks

The official title is the “Texas State Board of Examiners of Professional Counselors Supervised Experience Documentation Form.” Download the PDF from BHEC’s forms page or the upgrade-to-full-LPC page.5Texas Behavioral Health Executive Council. Your Identifying Information

The top of the form asks for your full name, the last four digits of your Social Security number, and your LPC Associate license number.6Texas Behavioral Health Executive Council. LPC Supervised Experience Documentation Form (Revised 5-4-2026) Match the name on the form exactly to the name on your license. A dropped middle initial or a hyphen spelled differently is enough to trigger a deficiency notice.

Supervisor Information

Your supervisor enters their name, email address, license type, jurisdiction of licensure, and license or registration number.6Texas Behavioral Health Executive Council. Hour Breakdown

The middle of the form splits your cumulative hours into three categories:

  • Total individual and group supervision hours, with the four-per-month floor honored throughout.
  • Direct client contact hours (face-to-face psychotherapy), at least 1,500.
  • Indirect experience hours, meaning supporting clinical work such as documentation, treatment planning, and consultation.

The figures on the form must reconcile with your monthly logs. Sit down with your supervisor and check the math before either of you signs. Small arithmetic gaps between your records and the totals reported are a common cause of delay.6Texas Behavioral Health Executive Council. If You Had More Than One Supervisor

Each supervisory arrangement gets its own form. A supervisor can only attest to the hours they personally oversaw, so hours from different supervisors cannot be combined onto a single SED Form. Prepare a separate form for each supervisor and each site.

The Other Three Items You Submit With It

The SED Form is one of four items in the upgrade package:8Texas Behavioral Health Executive Council. Upgrade to Full LPC

  • Texas Jurisprudence Examination certificate, completed within six months of submitting your upgrade application. If you passed it earlier than that, you will need to retake it.
  • The signed Supervised Experience Documentation Form.
  • A National Practitioner Data Bank (NPDB) self-query report, which you request yourself from the NPDB. BHEC accepts a certified electronic version uploaded with your application or emailed to npdb@bhec.texas.gov. A sealed hard copy can also be mailed to BHEC LPC Transcripts, 1801 Congress Ave, Ste. 7.300, Austin, TX 78701.
  • Fingerprinting, only if you did not complete it when you first became an Associate. If it is needed, BHEC emails instructions after you submit the online application, and you pay the fingerprinting facility directly.

Submitting Through the BHEC Portal

Log into your BHEC online licensing account, open the upgrade application, and attach each document as a PDF. Make sure scans are legible; blurry signatures and cut-off pages are common reasons for deficiency notices. The upgrade application itself carries no fee.1Texas Behavioral Health Executive Council. Your supervisor has a separate obligation: they must submit accurate documentation to BHEC within 30 days of the end of your supervision or the completion of your required hours, whichever happens first.2Legal Information Institute. 22 Texas Administrative Code 681.93 – Supervisor Requirements Coordinate timing so your application and their submission land at BHEC close together.

How Long Review Takes

BHEC’s published FY2025 data puts the median processing time for an LPC upgrade application at 8 days and the average at 22 days. The maximum recorded was 587 days, which reflects outlier cases with complications rather than typical experience. BHEC asks that you wait at least six weeks before contacting them about status.9Texas Behavioral Health Executive Council. Current Application Processing Timelines

If something is wrong with your submission, BHEC sends a deficiency email to the address on file with your account. Answer quickly; unresolved deficiencies are what pushes an application into the long tail of that timeline.

What Can Invalidate the Hours You Report

A signed Supervisory Agreement Form must have been submitted to BHEC before supervised practice began, and your supervisor is required to keep a signed and dated copy in your supervisory file.10Texas Behavioral Health Executive Council. LPC Rulebook 2025 – Section 681.93 Supervisor Requirements If it was never filed, hours under that supervisor may not count, and it is not something you can fix after the fact.

Your supervisor also needs to have been an LPC with current BHEC-approved supervisor status (LPC-S) throughout. They cannot be related to you within certain family degrees, and they cannot be your employee.7Texas Behavioral Health Executive Council. LPC Rulebook 2025 – Section 681.93 Supervisor Requirements Session-by-session records kept by your supervisor are what backs up the totals on the SED Form, so make sure they match before submission.

Texas Occupations Code Section 507.352 authorizes administrative penalties of up to $5,000 per violation, with each day a violation continues counting as a separate offense. Inflated hour totals or fabricated supervision records can lead to fines, suspension, or revocation for both the Associate and the supervisor who signed the form.
