The USYS medical release form, formally the Parent/Guardian Consent and Player Medical Release Form, is a one-page document you download from US Youth Soccer, fill in with your child’s contact, insurance, and medical details, sign as a parent or legal guardian, and hand to your team manager with a copy of your insurance card before the first practice or game of the season.

What to Have Ready Before You Start
Pull everything together first. It’s a short form, but it asks for information from several places.
- Your child’s full legal name, date of birth, gender, and home address.
- Two parent or guardian contacts and two additional emergency contacts, each with home and work phone numbers.
- Your health insurance company name and phone number, the policyholder’s name, the policy number, and the group number.
- Photocopies of both sides of your insurance card, which the form requires you to attach.1US Youth Soccer. Parent/Guardian Consent and Player Medical Release Form
- All known allergies and any other medical conditions.
- A separate written notice describing any additional issue, condition, or ailment beyond what fits in the allergy and condition fields. The form requires this notice to be attached.1US Youth Soccer. Parent/Guardian Consent and Player Medical Release Form
The insurance card copy and the written medical notice are the pieces parents most often forget. Without them, the form is technically incomplete.
Filling Out Each Section
The form has three functional parts: player and emergency information at the top, insurance and medical data in the middle, and the consent and waiver language at the bottom. Write clearly. Paramedics and emergency room staff need to read allergy information at a glance.
Player and Emergency Contacts
Enter your child’s full legal name, date of birth, gender, and home address. Then list two parent or guardian contacts and two backup emergency contacts, with home and work phone numbers for each. Those backup contacts matter. If you’re unreachable during a tournament out of state, the coach needs someone else to call.
Insurance and Medical Information
Fill in the insurance company name and phone number, the policyholder’s name, and the policy and group numbers from your card. Staple the photocopies of both sides of the card to the form.1US Youth Soccer. Parent/Guardian Consent and Player Medical Release Form In the medical fields, list every known allergy and condition. Don’t leave these blank. Write “none” if there’s nothing to report, because a blank field and an intentional “none” look identical to a first responder who has to decide fast.
If your child has asthma, a seizure disorder, a severe food allergy, or any other condition that could affect play or require special attention, attach the separate written notice the form asks for.
Consent and Liability Waiver
The bottom of the form contains two authorizations. The medical consent gives permission for an athletic trainer, licensed physician, or dentist to provide your child with medical assistance, treatment, diagnostic procedures, operative procedures, and X-ray treatment. By signing, you also agree to be financially responsible for the reasonable cost of that care.1US Youth Soccer. Parent/Guardian Consent and Player Medical Release Form
The liability waiver is separate. It releases USYS, its member organizations, sponsors, employees, volunteers, and facility owners from claims arising from your child’s participation in USYS programs, including transportation to and from those programs.1US Youth Soccer. Who Can Sign, and Whether You Need a Notary
Only a parent or court-appointed legal guardian can sign the form. A grandparent, an older sibling, or a family friend who drives to practice every week cannot sign unless they hold legal guardianship.
USYS itself does not require the medical release form to be notarized.3USA of Indiana. USYS Medical Release Form Individual tournaments sometimes add their own notarization requirement, so check the event’s website before you travel, or ask your club registrar which upcoming tournaments need a notarized copy. Many banks and shipping stores offer notary services, and some do it free for account holders.
Turning It In and Keeping a Copy
Hand the completed, signed form, the attached insurance card copies, and any written medical notice to your team manager before the first team activity of the season. Most clubs keep these forms in a physical team binder that travels to every practice, game, and tournament. If your child gets hurt, the coach hands the form directly to the paramedics or emergency room staff.
Keep a copy for yourself. A photo on your phone works in a pinch, but a scan or PDF in cloud storage is more reliable. If your club uses GotSport, the digital copy also lives there and coaches can print it from any device with internet access.4GotSport Help Center.
