How to Find and Fill Out Scantron Form 4521

To fill out a Scantron Form 4521 correctly, use a No. 2 pencil to completely fill in each bubble, bubble your ID number accurately in the identification grid at the top, keep the sheet flat and free of stray marks, and erase any changes thoroughly before turning it in. The form holds 200 questions across its front and back, with five answer choices per question labeled 1–5 and A–E.1Florida International University Bookstore. Scantron Form 4521 Everything else about getting a clean score comes down to a few habits the scanner cares about.

What You’re Looking At

Form 4521 is an 8½-by-11-inch, two-sided answer sheet. Questions are split across the front and back, roughly 100 per side, and each question is a single row of five bubbles.1Florida International University Bookstore. Scantron Form 4521 At the top of the front side you’ll find fields for your name, the subject, the date, and your ID number.

Along the left edge of each side runs a column of small black bars. Those are timing marks, and the scanner uses them to locate every row on the page.2Remark Software. Remark Classic OMR User’s Guide Treat the left margin as off-limits. Anything you write, doodle, or smudge near those bars can throw off the read for an entire row.

The Pencil and the Mark

Use a No. 2 pencil. Not a pen, not a marker, not a mechanical pencil with the wrong lead. The graphite in a standard No. 2 reflects infrared light in the way the scanner is calibrated to detect, and Form 4521 specifically requires a pencil-read scanner.3Scantron. 4521 GPAS 200Q (500/PKG) An ink mark may be invisible to the machine. A faint mark may not register at all.

Fill each bubble completely. The mark should be dark, solid, and inside the oval. A check, a dot, a slash, or a bubble you only half-shaded will often score as blank. If you’ve got a habit of putting a quick tick inside the oval, drop it for this form. The scanner is looking for a filled shape, not a signal that you meant one.

Pick one answer per question. Two filled bubbles on the same row usually scores wrong, not as a judgment call the machine makes in your favor.

Bubbling Your ID

The ID section is a grid of digit columns. Write your ID number in the boxes at the top, then fill the bubble for each digit in the column below. Work from the far left column to the right — the field is left-justified, so unused columns on the right stay blank.4University of Arkansas. Test Scanning Guidelines

Get this right. The scoring software matches your bubbled ID against an enrollment roster, and if the two don’t line up, your score has nowhere to attach. Some institutions run ParScore, which expects an ID of a specific length; a mismatch there triggers an error at the scanner.5Canvas Resource Center. How Do I Change the Student ID Length for ParScore Before you hand the sheet in, read the digits you’ve written across the top, then confirm the bubbles underneath match them column by column.

Changing an Answer

If you need to switch answers, erase the old one completely. A clean, high-quality eraser matters here. A partial erasure leaves graphite the scanner can still see, and it may read the leftover smudge as a second response — which typically scores wrong rather than registering as a double mark you clearly intended.6Angelo State University. Press firmly enough to lift the graphite, but not so hard that you crease or tear the paper. A torn sheet can jam the feeder, and a crease across the timing marks can misalign the whole scan.

Mistakes That Cause Misreads

Most problems with Form 4521 come from the same short list. Running through it before you turn the sheet in catches almost all of them.

  • Stray marks near the left margin. Any graphite close to the timing bars can cause the scanner to misread the row they belong to. Keep notes, doodles, and resting pencil tips away from the edges.7Salisbury University. Using Scantron (ParScore) Forms for Test Sheets
  • Incomplete erasures. A faint leftover competes with your intended answer, and the machine can’t tell which one you meant.
  • Physical damage. Folds, creases, tears, and staple holes block the scanner’s light path or misalign the feed. Don’t fold the sheet to fit a pocket or binder — carry it flat.
  • Wrong pencil. Pens and non-graphite marks may be invisible to the sensor.
  • Misaligned ID digits. Starting in the wrong column or leaving a gap in the middle breaks the roster match.
  • Light or partial fills. The oval needs to be solidly shaded, not sketched.

One thing to put aside: there’s no fixed percentage penalty for a stray mark. What actually happens is that an errant mark near a row can register as an answer choice you didn’t pick, and if that choice doesn’t match the key, that single question is scored wrong. The damage depends on which row is affected, not on any set formula.

Where to Get the Form

College bookstores typically carry Form 4521 individually, often for a dollar or less per sheet, which is the usual route for a student picking one up before an exam.8University of Hawaii Manoa Bookstore. Scantron Price List Departments and institutions that need volume can order from Scantron’s online store, where the form is sold in packages of 500.3Scantron. 4521 GPAS 200Q (500/PKG) Bulk pricing generally runs somewhere between roughly $290 and $580 for 500 to 1,000 sheets, depending on the vendor and any institutional discount in place.

Buy a spare if you can. If you ruin the first one with a tear or a bad erasure, having a second sheet means transferring your answers rather than arguing with the scanner.
