The Praxis fee waiver request form is a one-page PDF from ETS that eligible education students submit by email, along with a FAFSA Submission Summary and an Enrollment Verification Certificate, to receive a voucher covering up to three Praxis Core tests or one Praxis Subject Assessment per testing year.1ETS. Who Qualifies
Three conditions all have to be true. You must be currently enrolled in an undergraduate or graduate degree program; holders of a master’s or doctoral degree are ineligible even when pursuing a new credential in a different field. Your 2025–2026 FAFSA Submission Summary must show an Expected Family Contribution of $3,000 or less. ETS treats that figure as a hard cutoff, with no sliding scale or case-by-case income review.1ETS. Teacher Certification Test Fee Waivers and Eligibility
Only one waiver is granted per testing year. ETS also caps the number of waivers available per test date and per institution, and reviews requests first-come, first-served. Funds can run out before a testing period closes.2ETS. Praxis Fee Waiver Request Form
If you cannot file a FAFSA (for example, as a DACA recipient or international student), the ETS fee waiver page does not list alternative documentation. The FAFSA Submission Summary appears to be the only accepted proof of financial need.1ETS. Teacher Certification Test Fee Waivers and Eligibility
What the Voucher Pays For
An approved waiver covers either up to three Praxis Core tests or one Praxis Subject Assessment in a given testing year, not both. If you’re taking all three Core subtests (Reading, Writing, and Mathematics), ETS issues a single voucher tied to the combined test code 5752. The Praxis Core Combined test costs $150.1ETS. The waiver applies to the base test fee. ETS does not indicate that service charges such as late registration or rescheduling are included.1ETS. Teacher Certification Test Fee Waivers and Eligibility
Documents to Gather First
Have these ready before you open the form. A missing or illegible item will stall or sink the request.
- A complete copy of your 2025–2026 FAFSA Submission Summary showing an EFC of $3,000 or less. Download it from your studentaid.gov account. ETS also uses the older name, Student Aid Report (SAR), for the same document.2ETS. Praxis Fee Waiver Request Form
- An Enrollment Verification Certificate from your school’s registrar. It must carry a school seal, a National Student Clearinghouse watermark, or your registrar’s signature. A plain student-portal printout without one of those markers is not accepted.2ETS. Praxis Fee Waiver Request Form
- The specific test code for the Praxis exam you plan to take. You can look this up on the ETS Praxis website by state requirement or subject.
Filling Out the Form
Download the PDF from the registration and fee waivers section of the ETS Praxis website. It’s one page with four groups of fields.2ETS. Praxis Fee Waiver Request Form
At the top, print your last name, first name, and middle initial, followed by your mailing address, daytime phone, and email. The form does not request a Social Security Number.
Next, enter the test code for the Praxis exam you intend to take and check the box for your testing period. ETS runs four windows: September–November, December–February, March–May, and June–August. You may check a second box as a backup window, which is useful because funding can run out for your first choice.2ETS. Praxis Fee Waiver Request Form
Select your current education level. Provide the name of the institution or agency that requires your Praxis scores (typically your state department of education or your educator preparation program), and the name of the college or university where you’re enrolled. ETS uses four-digit institution codes to match enrollment records, so have that code on hand when you register with the voucher later.2ETS. Praxis Fee Waiver Request Form
Where to Send It
Email the completed form, your FAFSA Submission Summary, and your Enrollment Verification Certificate as attachments to a single message addressed to PraxisFeeWaiver@ets.org.2ETS. Praxis Fee Waiver Request Form
Send it at the start of your intended testing window. Because waivers are limited per test date and per institution and reviewed first-come, first-served, applications that arrive mid-period or later often meet an empty budget.1ETS. Teacher Certification Test Fee Waivers and Eligibility
Check that each attachment is legible and complete. A FAFSA page that cuts off the EFC, or a blurry Enrollment Verification Certificate, is a common reason for delay or denial.
After You Submit
If the request is approved and funds remain, ETS emails you a voucher with a unique code. Do not register or pay for your Praxis exam before the voucher arrives; the waiver cannot be applied retroactively to a test you’ve already paid for.1ETS. Teacher Certification Test Fee Waivers and Eligibility
When the voucher comes through, register as normal on the ETS website and enter the code at checkout to zero out the test fee. For all three Core subtests on one waiver, register under the combined code 5752, which is the code your voucher is tied to.1ETS. Teacher Certification Test Fee Waivers and Eligibility
ETS does not publish a review turnaround. Build in several weeks before your registration deadline so you have room for a slow response or a request for additional documentation.
