Who Can Use This Form
The program is open to SHBP members and their covered spouses enrolled in a non-Medicare Advantage commercial plan. Eligible 2026 options include the Anthem HRA, Bronze, Gold, and Silver plans, the Anthem Blue Cross and Blue Shield and UnitedHealthcare HMO plans, and the UnitedHealthcare High Deductible Health Plan.3State Health Benefit Plan. Where to Download the 2026 Form
You can get the form two ways. Visit BeWellSHBP.com and use the download button on the biometric screenings page, or go directly to the WiseTREND portal at psf.wisetrend.com/shbp.4WiseTREND. To Obtain a 2026 Physician Screening Form The WiseTREND site will ask for your first name, last name, date of birth, zip code, and gender to generate a form tied to your account. Print it and bring it to your appointment so your provider records the results directly on it.
What the Form Must Contain
The screening has two parts: measurements taken in the office and a blood panel. Your provider handles both, though the lab work may go to an outside lab and be filled in after.
Biometric Measurements
Your provider records your height, weight, the calculated Body Mass Index, and systolic and diastolic blood pressure.5Be Well SHBP. Biometric Screenings These values should be exact, not rounded or estimated.
Blood Work
The lab panel must include total cholesterol, HDL cholesterol, and fasting glucose.6Quest Diagnostics. Biometric Screenings A 9-to-12-hour fast before the draw is standard for accurate glucose and lipid readings. Ask when you schedule so you know whether to skip breakfast.
Provider Information and Signatures
The clinician must be a licensed Doctor of Medicine, Doctor of Osteopathic Medicine, Physician Assistant, or Nurse Practitioner. The form requires their National Provider Identifier (a 10-digit number assigned to every healthcare provider), practice name, address, and phone number.7Centers for Medicare & Medicaid Services. How to Submit the Completed Form
You have three submission options, all weighted equally:

- Mail the original to the return address printed on the form.
Keep a copy of the signed form and any confirmation number or fax receipt in case something goes wrong during processing.
Why Forms Get Rejected
The most common problems are a missing provider signature, an incomplete NPI field, or a screening date outside the January 1 through November 30 window.1Be Well SHBP. Biometrics Archives If your form is rejected, get the missing information from your provider and resubmit.
If a value is wrong, say your provider transposed digits in a blood pressure reading, ask the office to correct and re-sign the form, then resubmit the corrected version through any of the three channels.
Other Ways to Earn the Same Credit
The physician screening form is not the only path. SHBP offers two alternatives that skip the form entirely:
- Walk into a participating Quest Diagnostics Patient Service Center and complete the screening there. No physician appointment needed.3State Health Benefit Plan. Wellness
- Attend an SHBP-sponsored onsite screening event. Dates and locations are posted at BeWellSHBP.com.
Both feed results directly to Sharecare with no paperwork to submit. The physician form is most useful when you want to combine the screening with a regular checkup, or when no Quest location or onsite event is convenient.
After You Submit
Log into BeWellSHBP.com to check whether your screening has been processed and your 120 points posted. If the points do not appear after 30 days, or if you have questions at any stage, call Sharecare Member Services at 888-616-6411 (TTY: 711).8State Health Benefit Plan.
