Sports Car Club of America (SCCA) Forms: Waivers, Licenses, and Logbooks

SCCA forms fall into a handful of categories — membership, licensing, medical, waivers, vehicle paperwork, official credentials, and incident reports — and most of them live as downloadable PDFs at scca.com/downloads, with digital submissions handled through the Member Access Portal at my.scca.com. Physical packets that still need to be mailed go to SCCA Member Services, 6620 SE Dwight St., Topeka, KS 66619. Getting the right forms in on time, especially the medical exam and annual waiver, is what keeps you from being turned away at registration.1Sports Car Club of America. Membership Application

Every other SCCA form asks for a member number, so this one comes first. As of January 1, 2026, national individual dues are $85, up from $80. Regional dues add another $10 to $20 depending on where you live, putting the total at roughly $95 to $105 a year.2Sports Car Club of America. 2026 on the Horizon Family memberships run $120 nationally plus those same regional dues.3Sports Car Club of America. Why Become a Member

Two discounts exist. First Gear membership, for ages 21 to 24, knocks $35 off the individual rate.3Sports Car Club of America. Why Become a Member Active-duty military pay $50 for national dues but must submit proof of service; the discount doesn’t extend to spouses or dependents.4Sports Car Club of America. Novice Permit and Competition License

Racing under an SCCA sanction runs through two forms filed in sequence. The Novice Permit Application comes first. Anyone 14 or older who holds a driver’s license or has prior racing experience may be eligible.5Sports Car Club of America. I Want to Road Race

What you need to submit with the permit application:

  • A completed Competition License Physical Exam Form dated no more than six months earlier.
  • A photocopy of both sides of your driver’s license, passport, government-issued ID, or birth certificate.
  • Two online driver’s school modules, which take about 40 minutes total.
  • The novice permit fee. Call Member Services at 800-770-2055, ext. 215, for the current amount.
  • For ages 14 to 17, a digitally completed Parental Consent, Release and Waiver of Liability plus a Minor’s Assumption of Risk Acknowledgment.
  • For ages 18 to 21 in Alabama, Nebraska, or Wyoming, a notarized Minor Waiver Release.

Submit through the Member Access Portal or mail the packet to Member Services in Topeka.5Sports Car Club of America. I Want to Road Race

Upgrading to a Full Competition License

After the novice permit is issued, you complete one SCCA driver school and three additional race weekends within two years. At the third race, the Race Director or Chief Steward signs off the permit. Send that signed permit, a copy of your physical exam, a completed Competition License Application, and the license fee to Member Services. Drivers coming in with significant experience from another sanctioning body may have some or all of the on-track requirements waived by the Chief Steward at the driver school or by the Divisional Licensing Administrator.5Sports Car Club of America. Physical Exam Form

The Competition License Physical Exam Form has to be completed by a licensed physician and is the single document most likely to hold up your season if you forget about it. Renewal intervals shorten with age:6Sports Car Club of America. Examination and Medical History Forms

  • Under 40: every five years.
  • 40 to 49: every three years.
  • 50 to 69: every two years.
  • 70 and older: every twelve months.

It also includes a health history questionnaire covering vision, cardiovascular conditions, neurological issues, and anything that could impair motor function. Fill it out honestly. Emergency crews at the track rely on it, and an undisclosed condition surfacing mid-event creates liability problems for you and the organizers. Liability Waivers

Nobody enters a hot area — grid, course, pit lane — without a signed waiver on file. You have two options: a digital Annual Waiver through the Member Access Portal, or a paper waiver signed on-site at each event. For 2026, the digital option is available to both adults and minors and expires December 31.7Sports Car Club of America. Make 2026 Easy with a Digital Annual Waiver and Digital Membership Card

Submitting the Digital Annual Waiver

Log in at my.scca.com, go to the Online Store tab, and choose Annual Waiver Adult or the minor version. You’ll upload a photo (PNG, PDF, GIF, or TIFF, 3 MB or smaller) that clearly shows your face without hats, sunglasses, or masks.8Sports Car Club of America. Got a Minute? Submit Your SCCA Digital Waiver Before the Season Gets Rolling Complete the prompts and the waiver sits on file through the end of the year. Paper waivers at the gate still work; the digital version just saves time at every event you attend.

Minor Waivers

Waivers for participants under 18 require both parents or legal guardians to sign. If signed at the event, an SCCA official must witness and countersign. If signed off-site, the form must be notarized or witnessed by an SCCA official. A single-parent signature is acceptable only when that parent holds full custody with no other guardian having any claim. Paper minor waivers for non-members cover a single event and must be reprinted and re-signed for each one.9Cincinnati Region SCCA. Minor Waivers

Vehicle Logbook and Annual Tech

Every car competing under SCCA sanction needs a vehicle logbook, a permanent record of the car’s identity, safety equipment, and repair history. The Application for a New Vehicle Logbook captures chassis number, engine displacement, suspension type, and roll cage specifications. Inspectors use the logbook at later events to verify the car matches its documented configuration and that no unreported structural work has been done. Roll cage tubing specifications, including material and minimum diameters by vehicle weight, are set out in the SCCA General Competition Rules and reviewed at logbook issuance and tech.10Sports Car Club of America. Race Experience – Vehicle Requirements

Before the season, cars go through an Annual Technical Inspection. Signing the annual tech application certifies that you know the pre-event tech procedure and will inspect your own car before each event. Significant in-season changes — swapping an intake, changing suspension components, moving classes — require you to notify the Chief of Tech and update your annual tech. A mechanical failure on course triggers a mandatory renewal before you can compete again.

If you lose a logbook for an SRF, FE, or SRSCCA car, the National Office doesn’t keep records of logbook numbers stamped on roll cages. Contact SCCA Enterprises at (303) 693-2111 or enterprises@scca.com for a replacement.11Sports Car Club of America. Homologation Forms

Homologation Certificates

Formula cars and Sports Racers that don’t meet the standard roll-over structure rules in GCR Section 9.4.5 need a homologation certificate before they can compete. Cars with carbon chassis or non-standard roll-over structures submit the homologation packet to the National Office along with FIA paperwork or registered engineering documentation confirming compliance with GCR Section 9.4.5.F. If the make and model has already been homologated, that engineering paperwork may not be necessary.11Sports Car Club of America. Homologation Forms

Fees:

  • Initial homologation: $100 per chassis.
  • Duplicate certificate: $200.
  • Conversion fee: $50 per chassis.
  • Dual homologation: $100 per class.

The original or a copy of the certificate has to travel with the vehicle logbook to every event. For a previously approved design where the mailed certificate won’t arrive in time, the SCCA Technical Services Department can issue a two-race letter authorizing you to compete while the permanent certificate is in transit. Standard fees still apply.11Sports Car Club of America. Homologation Forms

Race Official License Application

Corner workers, timing staff, tech inspectors, and other officials file a Race Official License Application to receive insurance coverage and formal recognition.1Sports Car Club of America. Road Racing Forms and Documents The application asks you to pick a specialty: Flagging and Communications, Timing and Scoring, Scrutineering, Race Control, Registration, Starters, Sound Control, or Radio Tech.

Regional licenses are issued through your local region. Divisional upgrades require a regional license showing at least four events worked, with the Chief’s signature on your participation log. National upgrades require at least six events in a year plus approval from both the Regional Administrator and the Divisional Administrator. Renewals at any level require a photocopy of your participation log with the Chief’s signature.

Incident Report Form

When something goes wrong on track, SCCA officials complete an Incident Report Form. Three categories trigger a mandatory report: actual or potential bodily injury; significant vehicle incidents such as rollovers, impacts, fires, or mechanical failures with property damage; and damage to venue property like barriers or light poles.12Sports Car Club of America. Incident Report Form and Instructions

The form captures the specific location on the facility (turn, straight, pit, paddock), whether the person involved was a driver, crew, official, or spectator, the nature and severity of any injury, vehicle class and car number, and a written narrative. Anyone injured should also receive an Accident Medical Insurance Claim Form. For spectator injuries, fatalities, or serious participant injuries, officials must collect witness contact information.12Sports Car Club of America. Incident Report Form and Instructions
