Forms and Transactions You can fill in the fields and sign electronically through TIAA’s DocuSign experience, or download the PDF to print. You can also call TIAA at 800-842-2252 to request a paper copy by mail.

How to Fill Out the Form

Form F82 has three sections. Leaving any one incomplete slows processing.

Account Numbers

List every account that needs the update: TIAA annuity number, CREF annuity number, TIAA policy number, brokerage account number, and mutual funds account number. Enter the institution name — the employer or organization that sponsors your plan. Check your most recent statement or your online account if you’re unsure of a number.7TIAA. Former Name and Identification

Print your previous first, middle, and last name, then sign using your old signature. Enter your Social Security number so TIAA can link the change to your tax records and verify your identity. Skipping the SSN gets the form rejected.7TIAA. Notice of Change of Name

New Name and Reason for Change

Print your new first, middle, and last name, and provide your current mailing address. Check the box for the reason: marriage, divorce, adoption, court order, or other. If you choose “other,” write a brief explanation. Enter the date the name change took effect. For divorce and court-order changes, you’ll also list the court name and address.7TIAA. Sign in black or dark blue ink if you’re mailing paper. For the online version, sign through TIAA’s built-in DocuSign tool. TIAA does not accept digital signatures created through third-party programs like Adobe Acrobat.7TIAA. Notice of Change of Name

Supporting Documents TIAA Accepts

Include one piece of supporting identification with the form. TIAA accepts any of the following:7TIAA. Notice of Change of Name

  • Marriage certificate (a certified copy from a county clerk or state records office, not a decorative commemorative version)
  • Divorce decree showing the court’s authorization of the name change
  • Adoption certificate
  • Court order, for standalone legal name changes not tied to marriage or divorce
  • Birth certificate
  • Baptism certificate
  • Current driver’s license
  • Passport, valid or expired
  • Military identification
  • Will

For mailed submissions, TIAA requires originals or certified copies (stamped by the issuing agency or notarized) and returns originals after processing. One exception: death certificates, expired driver’s licenses, and qualified domestic relations orders are returned only if you include a separate letter asking for the document back.7TIAA. If you’re using the DocuSign version, upload a legible scan or photo during the signing step. The image must clearly show any certification stamps or other legally identifying markers.7TIAA. Notice of Change of Name

How to Submit the Form

You can submit online or by mail. TIAA does not accept name change forms or supporting documents by fax.8TIAA. FAQs About Account Forms – Section: How Do I Change My Name on My TIAA Account(s)?

Online is faster. Complete the digital Form F82, upload your supporting document at the signing step, and TIAA will send a confirmation through your online account.

To mail the form, print and sign it, then package it with the original or certified supporting document. If the document arrives separately, processing stalls. Use one of these addresses:7TIAA. Notice of Change of Name

  • Standard mail: TIAA, P.O. Box 1259, Charlotte, NC 28201-1259
  • Overnight courier: TIAA, 8500 Andrew Carnegie Blvd., Charlotte, NC 28262

Overnight delivery with tracking is worth the extra cost when you’re sending originals.

Retail Mutual Fund Accounts Need an Extra Step

If you hold retail-class mutual fund accounts through TIAA (rather than employer-sponsored plan accounts), you need to send a letter of instruction stating the name change, either instead of or in addition to Form F82. The letter must include your signature in both your former and new name, plus a medallion signature guarantee. A medallion signature guarantee is a specialized stamp from a bank or brokerage that verifies your identity; a standard notarization doesn’t substitute for it.8TIAA. After You Submit

Log into your TIAA account periodically to check whether the displayed name has changed. If nothing updates and no correspondence arrives within a few weeks, call TIAA at 800-842-2252 to check the status and ask whether anything needs resubmitting.

Review Your Beneficiary Designations

A name change doesn’t automatically update beneficiary information. TIAA recommends reviewing and updating beneficiary designations periodically, which you can do online at TIAA.org/profile or by completing a separate Beneficiary Designation Form.9TIAA. Beneficiary Designation Form If a beneficiary’s name has also changed (a spouse who changed their name too, for example), updating that record now avoids confusion when a claim is actually filed.

Line Up Your Tax Records

Once TIAA and the SSA both show your new name, make sure your next tax return uses the same spelling that appears on your current Social Security card. If a W-2 or 1099 still shows your old name after you’ve updated with the SSA, ask the issuer for a corrected form. The IRS allows you to manually correct the name on the copies you use to file, but corrected forms avoid questions.1Internal Revenue Service. Name Changes and Social Security Number Matching Issues
