Transfer Request Form: Signatures, Submission, and Fees

A transfer request form is the document that authorizes moving money between accounts, shifting securities to a new brokerage, or reassigning an employee to a different role inside an organization. The fields differ by context, but every version asks the same two things: who you are, and where the money or the position is going. Fill it in from a recent statement or the official job posting rather than memory. A single wrong digit in a routing number can send funds to a stranger, and a wrong department code can bounce a workplace transfer back to your inbox weeks later.

What to Fill In for a Bank or Brokerage Transfer

The identity side of the form asks for your full legal name, your account number, and often your Social Security or taxpayer identification number. Banks are required to verify identity before processing certain transactions. For purchases of cashier’s checks, money orders, and similar instruments involving $3,000 or more in currency, the institution must verify your identity through account records or government-issued identification.1Office of the Law Revision Counsel. 31 USC 5325 – Identification Required to Purchase Certain Monetary Instruments

The destination side asks for the recipient’s bank routing number (a nine-digit code identifying their bank) and the specific account number. A wire transfer may also require the recipient bank’s name and address.2HelpWithMyBank.gov. How Do I Transfer Money From My Financial Institution to a Family Member or Friend? International transfers add a purpose code that categorizes the transaction, such as investment, payment for goods, or family support, so regulators on both ends can track cross-border fund flows. Your bank will provide a dropdown or code list for this field.

Check every number against a recent bank statement before you submit. Routing and account numbers look similar enough that one transposed digit can redirect your money, and recovering misrouted funds is far harder than entering the number correctly the first time.

What to Fill In for a Workplace Transfer

An internal transfer request in a corporate or government setting asks for your current job title, department, and employee ID, along with the position and department you want to join. Many organizations also ask for the hiring manager’s name in the target department and a brief explanation of why you’re requesting the move.

A missing field here usually just means the form comes back to you. The cost is time. Confirm the exact job title and department code against the internal posting or directory before you submit, because every round trip adds days to a process already measured in weeks.

Which Signature the Form Needs

Most electronic banking transfers accept a digital signature. Under the federal ESIGN Act, a signature or contract cannot be denied legal effect simply because it is in electronic form, so clicking “I agree,” typing your name into a signature field, or signing with a stylus all count, provided you intended to sign.3Office of the Law Revision Counsel. 15 USC 7001 – General Rule of Validity

Transferring securities you hold in physical form is different. For stock certificates, bonds, and similar instruments, you will almost always need a medallion signature guarantee rather than a standard notarization. A medallion guarantee confirms your identity, verifies that you have the legal authority to transfer the asset, and holds the guaranteeing institution financially liable if the signature turns out to be fraudulent. Banks, credit unions, and broker-dealers that participate in a medallion program can provide the stamp, but not every branch offers it, so call ahead.4Investor.gov. How to Submit the Form

Most financial institutions accept transfer requests through their online portal or mobile app. Navigate to the transfers or documents section, fill in the fields, attach any supporting documents, and confirm. The platform generates a confirmation number. Save it somewhere outside the app itself, because if a dispute arises later, that receipt is your proof the request went in on a specific date.

Some institutions and transaction types still require a paper form sent by mail. Use a mailing method with tracking so you have independent proof of delivery. Workplace transfers typically run through an internal HR portal, though some employers still use paper forms routed through your current supervisor before they reach HR.

How Long Processing Takes

Timelines depend on the method and whether the transfer crosses institutional boundaries.

If a financial transfer hasn’t arrived in the expected window, contact the sending institution first. They can trace the payment and tell you whether it is still processing, was returned, or landed somewhere unexpected.

Fees You Should See Before You Submit

Federal law requires financial institutions to disclose transfer fees before you commit. When you first set up electronic fund transfer services, the institution must disclose any charges for making transfers.7Office of the Law Revision Counsel. 15 USC 1693c – Terms and Conditions of Transfers For remittance transfers sent internationally, the provider must give you a written disclosure showing the exact fees and exchange rate before you pay, and a receipt confirming those details after you authorize.8Office of the Law Revision Counsel. 15 USC 1693o-1 – Remittance Transfers

Amounts vary. ACH transfers are often free or low-cost. Domestic wire transfers commonly carry fees of $15 to $30 on the sending side, and international wires run higher. If you don’t see a fee schedule before submitting, ask. The institution is legally obligated to provide one.

If Something Goes Wrong

You have 60 days from the date your bank sends the statement showing an error on an electronic fund transfer to report it. That covers the wrong amount, a transfer you didn’t authorize, or a missing transaction. The clock runs whether or not you actually open the statement, so check your accounts after submitting any transfer. Once you report the error, the bank must investigate and resolve it within 10 business days, with an option to extend to 45 days if it provisionally credits your account in the meantime.9Consumer Financial Protection Bureau. 12 CFR 1005.11 – Procedures for Resolving Errors

If a transfer request is denied, the institution should tell you why. Common reasons for financial transfers include mismatched account information, insufficient funds, or a hold on the account; correcting the issue and resubmitting usually fixes it. For workplace transfers, denials usually come down to timing, qualifications, or departmental needs, and HR can explain. For denials tied to credit, federal law requires the institution to provide specific reasons or notify you of your right to request them, so a vague letter is not the end of the matter.10Consumer Financial Protection Bureau. Thresholds That Trigger Reporting

Large transfers trigger federal reporting requirements that surprise people who weren’t expecting them. These are automatic compliance obligations, not accusations.

  • Financial institutions must file a Currency Transaction Report with the federal government for any cash transaction exceeding $10,000 in a single day, including deposits, withdrawals, and currency exchanges. The bank handles the filing.11FinCEN.gov. The Bank Secrecy Act
  • Any business that receives more than $10,000 in cash in a single transaction, or in related transactions over a 12-month period, must report it to the IRS on Form 8300.12Internal Revenue Service. Understand How to Report Large Cash Transactions
  • If you transfer money to another person as a gift exceeding $19,000 in 2026, the donor must file IRS Form 709. You will not necessarily owe tax, since the amount above $19,000 reduces your lifetime exemption, but the filing itself is mandatory.13Internal Revenue Service. Deliberately splitting a transfer into smaller chunks to stay under the $10,000 threshold is called structuring, and it is a federal crime regardless of whether the underlying money is legitimate. If you need to move $15,000 in cash, move it in one transaction and let the bank file its report.

Once a financial transfer is approved, your institution should provide a transaction summary showing the amount, any fees, and the completion date. Keep that summary for at least a year, both for tax records and in case a dispute surfaces later.
